Not quite sure what you mean by Label name but they request that you enter the Pick # or appropriate reference number on the submission form and to place a label on the sleeve/holder with the submission form number and line number, for example XXXXXXX-001.

The note is sent back in the appropriate sized holder but there is an added fee for the oversize and custom holders. You have to select those boxes if the size of the note warrants it. 

Imaging means that PMG will take a picture of your note and email it to you for a fee, I would personally skip that option. Don't check that box.
