1834 Bank of the United States note and submission form 1 1

I'm trying to fill out a submission form but I'm not sure what to fill out for the required "series" field. Also, I'm worried this might be a contemporary counterfeit, and am not sure if PMG will grade it. I tried finding the catalog # and can't find it anywhere as well. Any suggestions?

Thank you for reaching out to us. You can enter the date/year on the note in the series field. Unfortunately, I cannot identify or confirm the authenticity of notes via photos, and submission is required so that the graders can evaluate the item. Any note submitted without a catalog number will be subject to a $5.00 research fee.
