Serial Number Error on Label

Please provide instructions on how I should resubmit this note for correction. The Serial Number on the Label doesn't match the note. I've attached pictures. Also, this note has a Rotator Serial Number that should have noted on the label.

Good afternoon.  Thank you for your message.  If you are a member of our Collector's Society, you can send this back in using a regular submission form and just notate "Mechanical Error" at the top of the page and notate what the error is.  If you are not a member then please contact our customer service department to obtain a complimentary services form to use.  You can also send us a receipt for your shipping fees and we can reimburse you for those charges as well.  If you have any questions, you can reach our customer service department at 877-764-5570.  Thank you!
