Does a courtesy autograph affect the grade given a note; that is, is any deduction made for the fact the note is written on? Also, does the insert indicate that the note is autographed?

To answer your question, PMG does not change the grade on a note with a Courtesy Autograph. We put Courtesy Autograph in the pedigree field & Indicated who signed it on the back of the label.

Does it matter who autographed it? I have an older silver certificate autograped by Danny Thomas, can that be graded?

We will certify genuine US or World currency. Generally courtesy autographs are reserved for treasury officials and/or government officials.
